Job Description

Within the framework of the objectives and policies established by the Campus Administrator and Director of Health Services, provides direct assistance through the provision of oversight, training and scheduling of the C.N.A. Personal Attendants. Also provides managerial and service task assistance when the Campus Administrator and/or Director of Health Services are not on-site. Provides direction to staff and service delivery according to the operational procedures of the facility and in compliance with the State regulations for RCACs.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Supervision and oversight of all Personal Attendants in both the Assisted Living and Attended Care Programs.
  • Conduct, attend and participate in departmental meetings, in-services, training, and community events as required.
  • Provide scheduling of all Personal Attendants.
  • Assist Health Staff with scheduling difficulties and provide direct service support as required.
  • Observe and report events or occurrences as necessary to the Director of Health Services and/or Campus Administrator.
  • Adhere to the philosophy, policies and procedures as established by ProHealth Care Regency Senior Communities.
  • Acknowledge, comprehend and support the residents and the Resident Bill of Rights.
  • Maintain confidentiality of both residents and staff.
  • Maintain communication with the Director of Health Services and the Campus Administrator regarding issues with residents, families and/or staff.
  • Complete Health Staff payroll as scheduled.
  • Schedule all resident services into the Scheduler Database Program.
  • Print out and post Schedules as required for staff use.
  • Comply with all required local, state and federal regulations.
  • Attend and participate in staff and supervisory meetings, committee meetings, trainings, in services, community events as required.
  • Assess and document any change to the resident status or needs.
  • Supports and implements the planning, review, and evaluation of the care need of the residents and implements the care services through appropriate staffing. Creates or updates ISP's as needed.
  • Notify physician and/or responsible parties and primary contact of change and/or needs of resident in conjunction with and under direction of the Director of Health Services.
  • Respond to resident and staff emergencies quickly and in a calm and professional manner. Create incident reports as required and notify Director as needed. If staff requires medical attention for a work injury, make arrangements for staff to be seen at Concentra.
  • Conduct reports for attended and assisted living programs.
  • Receive and put away pharmacy deliveries.
  • Set up insulin, reorder insulin and syringes as needed.
  • Perform medications refill ordering.
  • Perform Med MOT fills, includes checking them in and putting them out to the floors on the correct day.
  • Process MD orders.
  • Perform follow-up on weekend report and incident reports.
  • Fax lab results as required.
  • Perform family calls for supplies as required.
  • Update MARs documentation in software.
  • Notify Director of needed supplies.
  • Perform Treatments.
  • Create risk agreements for identified risks of residents as necessary.
  • Perform weekend admissions of new residents as necessary.

  • All other duties as assigned

QUALIFICATIONS:

  • Must possess a current and valid Wisconsin Nursing License with two years experience preferably in the area of geriatrics.
  • Ability to read and understand, written or verbal labels and instructions in English.
  • Ability to work cooperatively as a support staff with all departments of the organization.
  • Attend and participate in departmental meetings, in-services, training, and community events as required.
  • Availability to work nights, weekends and holidays.
  • Comfortable assigning and/or performing tasks, jobs, and/or projects in conjunction with day-to-day activities.
  • Computer literate with accurate data entry skills, and familiar with Microsoft Word and Excel.
  • Effective communication and interpersonal skills with the ability to work harmoniously with supervisors, peers, and subordinates.
  • High energy level, well organized, and the ability to perform short-cycle, repetitive tasks.
  • Maintain professionalism in all communication and actions.
  • May be called in to work another shift to perform tasks as needed.
  • Must pass required pre-employment tests and screenings.
  • Possess valid and current licensure and/or certifications.
  • Possess valid and current driver's licensure and insurance.
  • Prior experience working with the geriatric population preferred.
  • Strong organizational skills with the ability to multitask, and set and enforce stringent standards.
  • Strong supervisory skills along with the ability to delegate and work harmoniously with all levels of the organization.
  • Strong interpersonal skills, and the ability to work unsupervised and/or harmoniously with all levels of the organization.

PHYSICAL AND MENTAL DEMANDS:

  • Discriminate colors in reading diagnostic slides or warning lights.
  • Able to concentrate on fine detail with constant interruption, and attend to task/detail for more than 50 minutes at a time.
  • Able to perform difficult manipulative skills such as calibration of equipment.
  • Able to remember multiple tasks/assignments over long periods of time and take notes as required.
  • Able to speak clearly. Verbal ability necessary to orient service options to residents, and/or family members with varying levels of comprehension.
  • Ability to distinguish smells.
  • Adequate vision and hearing with ability to distinguish sounds such as voice patterns, auscultation, tape recordings, etc.
  • Able to walk and stand for prolonged periods of time and are able to reach, twist, sit, push, pull, squat, climb stairs, reach above shoulders, and bend at the knees.
  • Creative communicator, well organized, and resourceful.
  • Good reasoning and sound judgement.
  • Must be able to lift 50 pounds.
  • Must have genuine interest in the needs of the elderly population, understanding the problems they encounter and have patience and tact in serving their needs.
  • Spend four to six hours on your feet throughout the course of the workday.
