Job Description
Major Responsibilities:
  • Provision of Patient Care
    • 1)Plans appropriate treatment programs for patients of all ages (depending on area of assignment) based on physician referral, evaluation results, patient/family goals and interests, sound clinical judgement and other pertinent information. Responsible for maintaining a productive schedule of 3.2 WLU's per productive hour worked. Alerts manager of significant openings and/or cancellations in their daily schedule.
    • 2)Administers daily treatment ensuring that the treatment plan is age-appropriate and adjusted as necessary in order to meet the changing needs of the patient. Reassesses patient's needs as appropriate.
    • 3)Successfully completes all competency assessments pertaining to provision of patient care (including annual completion of 2.0 hours of Stroke Education per DNV guideline).
    • 4)Communicates and collaborates with team members, other professionals, family members, and caregivers for the purpose of sharing information that will assist in achieving optimum care for the patient. Establishes effective rapport with patients, family members, and other professionals.
    • 5)Selects appropriate adaptive equipment for patients and provides appropriate instruction in the maintenance and use of such equipment.
    • 6)Maintains responsibility for patient plan of care and provides direction and recommendations regarding treatment and goals implemented by assistive personnel.
    • 7)Provides education to the patient and family/significant others based on patient needs. Identifies and addresses learning readiness, preferences, and learning barriers.
    • 8)Participation in community education programs and marketing is encouraged.
    • 9)With other team members, recognizes the need for termination of treatment and effectively plans discharge which may include establishing home treatment programs and arranging for equipment/devices.
    • 10)Maintains a safe environment for patients.
  • Patient Assessment
    • 1)Selects appropriate occupational therapy assessment procedures to assess the abilities and limitations of patients of all ages depending on area of assignment.
    • 2)Adminsters, analyzes, and interprets initial occupational therapy assessments which determine the patient's status in the following areas of function: locomotion, strength, endurance, cardiopulmonary function, balance, coordination, joint mobility, flexibility, pain, job related tasks, wound status, and functional mobility skills as related to patient activites of daily living.
    • 3)Establishes short and long term goals which are functional and measurable.
    • 4)Evaluates patient progress towards goals and expected outcomes.
    • 5)Completes all pertinent assessment competencies.
  • Documentation
    • 1)Completes all documentation according to department policy.
  • Daily Operations
    • 1)Performs other duties such as entering charges for all services rendered on the day delivered, scheduling patients, maintaining a clean work environment, and serving on departmental committees and program development efforts as appropriate.
    • 2)Promotes patient safety by reporting issues through established channels and participating in safety initiatives as requested.
    • 3)Participates in continuous performance improvement projects.
    • 4)Follows appropriate safety and infection control measures pertaining to patient care, equipment, and the general working environment. Promptly reports any problems to rehabilitation manager and follows Advocate Sherman Hospital policies and procedures.
    • 5)Actively participates in departmental meetings, committees, in-services, and continuing education programs.
    • 6)Delegates patients to assistive personnel with direction and ongoing supervision while remaining responsible for the patient's treatment program and goals.
    • 7)May supervise occupational therapy students at the request of the Rehab Department's Center Coordinator of Clincial Education after completing one year of clinical experience.
    • 8)Complies with organizational and departmental patient satisfaction initiatives. Routinely meets the needs and expectations of patients and their families.
    • 9)Upholds all policies and procedures of Advocate Sherman Hospital.
    • 10)Performance of other duties as assigned (i.e. supply ordering, equipment inventory, etc.).


Licensure:
  • Occupational Therapist


Education/Experience Required:
  • Bachelor's degree in Occupational Therapy or above from an accredited school.


Knowledge, Skills & Abilities Required:
  • -Good interpersonal and communication skills -Competent in a variety of diagnostic and treatment techniques -Recognizes needs and behaviors of specific age groups of patients treated
  • -OTR/L - Illinois Department of Professional Regulation. -CPR certification


Physicial Requirements and Working Conditions:
  • -May be exposed to hazardous materials and life-threatening diseases -Direct contact with body fluids -Pushing, pulling, lifting, and transferring patients -Ability to assist, escort, transfer, and respond to patient needs for extended periods of time -May be required to work evenings, weekends, and holidays (Full time/part time staff coming on board at Advocate Sherman Hospital have the minimum requirement of working one weekend shift per month and one holiday per year in the inpatient setting. Associates hired specifically for the outpatient setting must be available to work until 7:00 p.m. two evenings per week in adjunct to the inpatient rehab requirement aforementioned).
  • This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
