ANEW Home Health Care Est. 4/1983 ANEW Home Health Care has been a licensed and certified home heath care agency since October 30, 1984, under HSS 133. ANEW is in compliance with the State of Wisconsin, Department of Health and Family Services, Division of Health. ANEW provides Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, and Nutritional Guidance with a Registered Dietician, Medical Social Services, Nurses Aides, Personal Care Workers, Companions, Homemakers, and Respite Care Staff. Professional Relative Est. 2000 Professional Relative provides private duty services from nursing to caregiver and companions. ANEW CMU Est. 1/1999 ANEW Care Management Unit (CMU) provides community based long-term care management for members in the Family Care program. Family Care is a Medicaid waiver program, funded through federal and state tax dollars. In Milwaukee County, the Family Care program is administered through the Milwaukee County Department of Family Care serving eligible residents, age 18 and over. ANEW CMU is one of the largest CMUs in Milwaukee County and serves over 700 members on a monthly basis. All professional staff have at least a four-year college degree in a health related field and/or have an RN license.  Each Family Care member is assigned a Care Manager and a Nurse as part of his/her interdisciplinary team (IDT). Milwaukee Express Moving Est. 1/2001 Milwaukee Express Moving Company is a full service moving company which primarily services the frail elderly population throughout Milwaukee County and has been a Provider with Milwaukee County since 2001. Milwaukee Express staff provide an estimate to the customer prior to the move. The staff is experienced and skilled in packing, unpacking and moving. Milwaukee Express offers packing supplies at an additional cost to the customer. ANEW Heavy Duty Cleaning Est. 4/1990 ANEW Heavy Duty Cleaning is a full service cleaning company which primarily services the disabled and frail elderly population throughout Milwaukee County. ANEW Heavy Duty Cleaning provides deep cleaning services within a member’s home. The duties and tasks consist of usual vacuuming and dusting, washing baseboards, walls and floors, washing inside windows, sanitizing and disinfecting bathrooms, cleaning appliances, and general cleaning of the household. Highland Gardens Supportive Living (HGSL) Program Est. 10/2001 Highland Gardens Supportive Living (HGSL) is a Family Care funded supportive living program designed to assist members who qualify to live independently in a subsidized apartment, but who need frequent help through waking hours, but are safe to be alone during the overnight.  This program currently serves Family Care members who reside at the Highland Gardens complex. These residents each have their own independent apartment, but share the assistance of ANEW Home Health Agency Aides (CNA/PCW), who are on-site 7 days/week. These aides provide each member assistance with any personal cares, medication reminders, housekeeping, activities, and meal preparation, that is designated in his/her individual service plan.  ANEW Fiscal Agent Services Est. 1/2002 ANEW Fiscal Agent Services provides fiscal agent and Employment Broker services to Wisconsin residents.

SUPPORTIVE HOMECARE OPTIONS, INC. Est. 5/1997 MISSION AND PHILOSOPHY Supportive Homecare Options, Inc. (SHO) specializes in providing community-based supportive in-home services. SHO employs caregivers on behalf of the consumers to provide basic homemaking and personal care services. SHO assists in evaluating and developing individualized self-directive programs. It is our mission to foster each consumer's capacity to lead a meaningful and productive life by encouraging self-direction, promoting self-worth and supporting the retention of functional abilities. SHO respects the value and dignity of ALL. The primary role of Supportive Homecare Options, Inc. is to employ non-professional staff such as Personal Care Workers (PCW) and Supportive Homecare Workers (SHC) for the provision of ADL (Activity of Daily Living) and IADL (Instrumental Activity of Daily Living) assistance to the members we serve. These employees provide necessary assistance to the members in order to meet their daily living needs and to assure adequate functioning at home. These services may include personal care, chore services, routine homemaking/maintenance and supervision. SHO’s secondary role is to employ professional staff, such as Registered Nurses, for the purpose of completing a skilled assessment on the members we serve in order to assess the member’s functional status.

QUALITY HEALTHCARE OPTIONS, INC. Est. 6/1988 MISSION AND PHILOSOPHY To develop and provide QUALITY HEALTHCARE educational programs which are affordable for the entire community. To educate and train professional, paraprofessional and lay persons to provide healthcare skills with confidence and accuracy. To educate the community of the OPTIONS available to them to live independent and productive lives. As well as to establish an alliance within the community by offering fulfilling employment opportunities to ALL. Quality Healthcare Options, Inc. ("QHO") has been Wisconsin's largest state and federally approved Certified Nursing Assistant Instructional and Competency Program since 1988, when it became the first state approved program under the OBRA 87 requirements. QHO's overall mission is to educate and provide employment opportunities in the health care network. QHO's instructional staff have many years of nursing, professional and educational experience between them and demand the utmost capabilities of their students before they earn their certificate. QHO has trained Nursing Assistants for many area nursing homes, home health agencies, psychiatric hospitals, hospices, independents, Wisconsin Department of Vocational Rehabilitation, Wisconsin Job Service, Community Based Residential Facilities, referrals from Wisconsin's Bureau of Quality Assurance and for the W-2 Agencies. Since QHO's inception, thousands of CNAs have been successfully trained and competency tested at a rate of 99% passing throughout the State of Wisconsin. QHO's programs have expanded to provide continuing inservice education for health care providers and the community, annual skills evaluations for Certified Nursing Assistants, American Red Cross CPR and Basic First Aid program, American Heart CPR programs, QHO's own state approved Basic First Aid program. QHO also provides consultation services for basic literacy skills and job placement for the paraprofessional. CNA training programs have been offered in Russian, Spanish and Hmong. QHO has provided numerous transitional home-to-work training programs for the W-2 Agencies, Job Service Network and Wisconsin Division of Vocational Rehabilitation. QHO is a certified Emerging Business Enterprise (EBE) in the City of Milwaukee.

QHO FINANCIAL SERVICES Est. 10/2006 under Quality Healthcare Options, Inc. MISSION AND PHILOSOPHY To provide sound, trustworthy financial management and representative payee services to ALL consumers we serve. QHO Financial Services was established in October of 2006. QHO's Financial Services staff is experienced in working with the Family Care Program and understands the needs of the Family Care members and Care Managers. QHO Financial Services has become a well-established Representative Payee/Money Management company for Family Care. Representative Payee/Money Management services include: - Financial Managers that meet with members regularly to ensure that their needs are being met - Managing customer social security checks - Completing Title 19 reviews - Filing Homestead Credit when appropriate - Assisting members with financial management and budgets - Helping members organize finances - Balancing member's checkbook - Writing checks out for members QHO Financial Services has positive working relationships with the Social Security Administration and with Family Care Contract staff and the CMUs.

ANEW and Sun Starr Real Estate Group have developed a partnership in the operations of a 62 unit Assisted Living at Garden Place; an 80 unit Assisted Living at Garden Terrace, and a 60 unit Assisted Living at Becher Terrace. ANEW Management, LLC provides a full service package to the residents of Garden Place RCAC (Residential Care Apartment Community), Garden Terrace RCAC, and Becher  Terrace RCAC. ANEW employs the staff which are required for the operations of the support services. This package includes health care management, dining service, personal care, activities, housekeeping and laundry. 

ABUNDANT LIFE MANOR Est. 5/2006 MISSION AND PHILOSOPHY Abundant Life Manor is owned and operated by Abundant Life Ministry, Inc., a faith-based, non-denomination, non-profit corporation that seeks to enhance and enrich the lives of at-risk individuals and families through programs designed to support their housing needs, vocational development and life skills training. While nurturing individuals' capacity to think critically, creatively and skillfully; Abundant Life Ministry also strives to deepen their commitment to values such as kindness, helpfulness, personal responsibility and respect for others. These are qualities we believe are essential to transforming communities. Abundant Life Ministry affirms the value of human life and believes that through fostering supports and nurturing the human spirit individuals and communities will be enriched. Abundant Life Manor is owned and operated by Abundant Life Ministry, Inc. a faith-based, non-denomination, non-profit corporation that seeks to enhance and enrich the lives of at-risk individuals and families through programs designed to support their housing needs, vocational development and life skills training. Abundant Life Manor CBRF opened for occupancy on May 11, 2006. Abundant Life Manor is a 42 bed community based residential facility (CBRF) for older adults with behavioral health challenges including those emotionally disturbed, chronic mental illness, chronic alcohol and/or drug dependency, or cognitive impairments. Abundant Life Manor brings together supportive services, personal care and medical management designed to support the resident's needs. Our staff is available 24 hours per day, 365 days of the year. Abundant Life Manor's specialty programs will include: socialization skills, community integration "City of Milwaukee is the Center of our Activities", resident kitchen/cooking classes, support groups, work skills, music therapy and health care coordination programs. We will empower our residents in individual-self directive goals with outcome measurements. Abundant Life Manor believes in resident and staff centered mutual goal setting using the "STAR" method. The "STAR" method is an integrative care model which allows staff to take an integrated approach to resident care. Abundant Life Manor is staffed with a Registered Nurse, Behavioral Health Specialist, Social Worker, Life Enrichment Coordinator, Registered Dietitian, Dining Service Coordinator, Cook, Dietary Assistants, Resident Assistants, Housekeeping, and Maintenance. We have a minimum of two awake staff on at all times to assist with the residents' needs. Additional staff is available as dictated by the residents' needs as outlined in their individual service plan and according to CBRF regulations. All Resident Assistants are highly qualified Certified Nursing Assistants. Abundant Life Manor is licensed by the State of Wisconsin as a Class C Nonambulatory Community Based Residential Facility who may serve residents with advanced age, alcohol/drug dependent, and emotionally disturbed/mental illness.
